Accessing Your Records

You have the right to request a copy of your health care records.

  • You can ask your counselor during a session or email request to your counselor.

  • You’ll need to complete and sign a Release of Information form from this practice.

Sharing Records with Someone Else

 If you’d like your records shared with another provider (for example, another doctor):

  • You’ll need to request and complete and sign a Release of Information form from this practice.
